Export Forms Must Be Filled Out Correctly For U.S. Beef Entry

For U.S. beef to enter Hong Kong, it is imperative to fill out FSIS Form 9060-5 without errors, ensuring the establishment number and name and address information on Form 9060-5 match the information on the AMS Web site. Errors such as incorrect establishment numbers or names and addresses of establishments may result in delayed or denied entry.

If the product was certified at a slaughter/cutting plant, check both "slaughtering plant" and "processing plant" blocks in the section titled "Product Exported From."

The following information must be provided in the Remarks section of FSIS Form 9060-5:

(1) Establishment number, name and address of slaughter plant and slaughter dates in month/day/year format. Date ranges are acceptable.

(2) Establishment number, name and address of processing/boning plant and processing dates in month/day/year format. Date ranges are acceptable.

If this information is not provided in the Remarks section, the product will be denied entry. Additional certification statements in the Remarks section of FSIS Form 9060-5 are prohibited.

Japan

U.S. Beef Destined For U.S. Military Use Will Be Accepted

Although U.S. beef is not eligible for export to Japan, U.S. beef headed for U.S. military bases in Japan is still eligible. See the Export Requirements for Japan in the FSIS Export Library.
