Exporters MUST Use Current Version of 9060-5

Exporters to Taiwan should note that Taiwan is only accepting the most current version of the FSIS Form 9060-5, dated 12/14/2006.  This date is located on the lower left-hand corner of the form (see below). Product will be detained if older versions of the 9060-5 are used.

Additionally, if exporters have to use an “In Lieu Of” certificate for any reason, Taiwan is not allowing shipments to enter if the inspector’s signature on the In Lieu Of certificate is different from the inspector who signed the original 9060-5 certificate. USDA is working to sort this out.

Taiwanese inspectors are scrutinizing export documents, so members should ensure that all the information on export documents is correct, and the most current version of FSIS Form 9060-5 is used.  Please contact USMEF (303-623-6328) if you have additional questions.
